* Packaged in the 2000 revised version of the 1994 cover - it's mostly the same as versionthe #21995 revised version of the cover with the issue number ending with -4, except the Proof of Purchase tab was printed on the left flap underneath the top of the cover and the spines of the cover have rounded loop holes on the far bottom.
* Uses the 1994 horizontal silkscreen-inked label.
* Duplicated by Technicolor Video/CD/DVD Services in either Camarillo, California or Hollywood, California, using either a Sony Sprinter or an Otari TMD -- Inked printing on the tape spine. Barcode in the far right. Big number in the far left. Print/press date: April 11, 2001. No codes on the vertical interval at the beginning.
