* Packaged in the 2000 revised version of the 1994 cover - it's mostly the same as the 1995 revised version of the cover with the issue number ending with -4, except the Proof of Purchase tab was printed on the left flap underneath the top of the cover and the spines of the cover have rounded loop holes on the far bottom.
* Uses the 1994 horizontal silkscreen-inked label.
